Before your first visit, please complete the new patient forms listed below and bring all items listed to your appointment.

1. Read 2014 HIPAA Agreement (You will be required to sign the HiPAA Acknowlegment Form in Step 2 below).
2. Signed and Completed New Patient Intake Form. Click here to download.
3. Photo ID
4. Insurance Cards
5. Imaging (i.e: MRI, XRays, CT Scans): If obtained at the following imaging centers, we can access through our internal records system:
   • Centura Hospital System: (Ex: Porter Adventist, Littleton Adventist, Parker Adventist, St. Anthony North)
   • Health Images
   • Invision Sally Jobe Imaging Services
   • Touchstone Imaging
6. If not from one of these locations, please obtain a copy of the appropriate imaging on a disc format AND the associated radiology report for review during your consultation.
7. You may also fax your signed forms directly to our office @ 303-282-4407

We’re here to answer your insurance and other financial questions related to your treatments at SPARCC. We accept most major forms of insurance. All Payments and Co-Payments are due at time of service. Payment options may be available for self pay and uninsured patients.
